Battery is draining and not charging with charger plugged in

I noticed my phone getting warm and while plugged up to be charging, the battery is actually draining while being plugged up. The numbers are going down and not up. I did the soft reset in recovery mode, lost everything, and basically started over. The phone is still doing the same. If I leave it plugged in overnight, when I wake up 8 hours later it is still not fully charged. After a little use and I try to charge it, it charges very slow. I've went in developer options and did all of that. Also installed CPU Spy. I don't know if I am missing something or what. Can someone please assist me with this issue!?
 
It's one of two things - either the battery is bad (which can happen with a battery that's over 2 years old, especially if you haven't taken proper care of it - like never discharging it past 40%), or there's something in the phone drawing a lot of current (if the app draws more current than the phone is designed to draw from the charger, the net current is out of the battery, not into it).

Look in Settings/Battery and see what's drawing how much. Post a screen shot (see How To Post Screenshots on Android Central). If it is a bad battery, Note 4 batteries are only about $10 (look on Amazon).
 
I bought a new battery in August. This seemed to happen all of a sudden Wednesday (11-23) night.
I tried posting the screen shot but nothing happened when I plugged my phone to my computer other than the sound to let me know it is connected.
Under Battery/settings, it says Screen 8%, Android System 8%, Android OS 2%, Bluetooth 2 %, Cell Standby 2%, Google Services 1%....there are a few more but those are the top ones.
 
It could be that your charging port is damaged. The USB cable you are using could be damaged in some way.

Are you using the OEM charger unit that plugs into the wall? If you are using a third-party wall plug then it could be poorly made... I would suggest only buying reputable third-party wall plugs (Anker is good, Griffin, Belkin, etc are good too) or stick with OEM wall plugs.

Is your USB cable OEM or third-party? Same thing goes for your USB cable... Only buy reputable brands or stick with OEM.

You mentioned you bought a new battery, correct? Is the new battery really an OEM Samsung replacement or is it a cheap imitation? When buying replacement batteries, I would only buy OEM replacements.
 
Is your phone running warm or hot when its charging? Is it running hot while sitting inactive?
If its a bad battery typically the phone wont heat up as the battery doesnt have the capacity to fully charge. IF its running hot then the battery has the power to run the device but something on the device is not working properly.
