Big problem when installing Jelly Bean OTA

No locking the device didn't help for me. I get the same issue :(:(:(

does re-locking wipe the device???

Locking does not, but removing root does.

Unlocking wipes. Locking doesn't. And locking isn't required to receive or apply the OTA. The OTA doesn't care what your bootloader lock state is.

Apparently it does, because it didn't work when I was unlocked. I locked it and it worked.

Are you stock 4.0.4 and unrooted? Use WugFresh toolkit and flash back to stock and relock the OEM if not. It should work then.
 
Something else was your issue. I can 100% guarantee you that an OTA doesn't care if your bootloader is locked or unlocked.

Well it was root access then. But why would you have an unlocked phone without rooting it? I look at it as one in the same. (but I do understand the difference)

You can have an unlocked bootloader to manually flash updates if you don't want to wait for OTAs. Just because you unlock doesn't mean you have to root. You can remain stock.
 
After a few days I found that the issue is due to the radio on 4.0.4 being the same from an earlier build . If you dont have second version of radio XXLA2 that goes with 4.0.4 the OTA to JB will fail.

The phone can be unlucked during upgrade, but not rooted.

So the best is to do a Titanium backup on ICS, and then flash back to stock with the WugFresh Nexus root toolkit

Then do the forced OTA
