Call App Freezes (Doesn't Hang Up)

My wife and I both have a Note 5 on Verizon. We've both noticed an issue that happens intermittently when making/receiving phone calls. When it is time to end the call, the phone app will freeze. If I press the home button and then go back to try to make another call, the only options I have are to return to dialer and make emergency call. It will not allow me to make another call. It is as if the network is not letting the connection to. I had also downloaded the Google dialer to try it out and am having the same problem. I have already wiped the cache partition but haven't done a factory reset yet. Is anyone else having this issue?
 
It hasn't happened in safe mode on mine so far but it also doesn't happen all the time when it's not in safe mode. I'd say it probably happens 20-25% of the time not in safe mode.
 
Does it happen at least once a day? If so, then keeping the phone in Safe Mode (as inconvenient as that may be) can still give you a good idea.

It may also be worth talking to Verizon about this, in case it's a network issue.

It doesn't necessarily happen once a day. More like 2-3 times a week. Thought about keeping it in safe mode all the time but would really prefer not to. Here are some screen shots that show up.
