Call Limit Exceeded

My wife just called me to say whenever she does anything on her phone she is getting a notice that says "call limit exceeded".
Anyone else encounter this?

This happened to me twice this morning, I just clicked 'OK', everything seems to be working fine though. Not sure what this means.
