Calls straight to voicemail....not again

My boyfriend has a Sony Xperia TL with Android 4.1.2 Jelly Bean. He has been having an issue where incoming phone calls would ring and go to voicemail; however his phone does not ring. He will receive a voicemail notification immediately after somebody leaves a message. I have a Samsung Galaxy Express and in the same room the same would happen. I would call and receive 10 rings and then voicemail while his phone decides to give us the silent treatment.

I have been researching on the web about this issue and no such luck. Here is what we did:

1. Call forwarding: Disabled completely (Always forward Disabled, Forward when busy Disabled)
2. In power management we disabled the extended standby mode
3. In the battery option I noticed that cell standby "Time without signal" was a 69%. When I disabled extended standby mode and charged the phone for 10 minutes (battery went from 30% to 50% the standby "Time without signal" dropped to 55%. (In contrast my phone is at 0% and we are in the same approximate location)
4. Tried restarting the phone when changing settings are 80% of the time incoming calls ring and go to voicemail while the phone is silent. We drove across town to try it again and it only went down to 75% (I called at least 20 times and had a friend call his number a couple of times)

Currently we have AT&T 4gLTE service. Majority of the time we use WIFI instead of Cellular Data. There is one part of the city where there is a dead spot that we pass on our way to work, (it is only a 2 block radius and we received incoming calls) Now it has become a problem because this is also his work phone.

In two days we are going to try to go to AT&T to see what the issue is about. He thinks it is a sim card issue. If it was then when I re-set the simcard (take it in take it out deal) it still had the issues. (It worked for my phone, apparently the lady setting up my phone had it in crooked X_X).

Because the interface of the XperiaTL is different from my Samsung Galaxy Express I am scratching my head to find the answers.

What are your suggestions?!

Well, not to be the patronizing tech support guy, but have you checked that a working ringtone is set and the ring volume is turned up?

The time without signal number is worrying; it makes me wonder if there's a problem with his antenna or radio. Unfortunately, that sort of thing is over my head.
 
The ringtone is set to default and the ring volume is at max. When I called a new moments ago you can hear it ring across the house. It is very loud.
 
No he is not using any call blocking application. The only call application he has is the Phone App that was installed by default. Of course if he did not have this app he could not make calls.

He has a "as close to factory state" as humanly possible. All he has on there is music, photos, contacts and his email.

We re-set the sim card again and again calls are going to voicemail. Earlier today it was no so bad because we were on the other side of the city. But it is a problem when his boss has to reach him ASAP
