Cannot get 3G on Cincinnati Bell Wireless

Hi, I recently purchased a Samsung Galaxy Note 2 from Amazon. It is the international GSM model. I can get connected to the CBW network but only get Edge cannot get 3G and I also cannot get apps to download. I have seen other posts with users that have had the same issue but have yet to find a resolution. I have already had Bell give me a new SIM card, I have verified my APN settings with them, I have upgraded the software on the phone and I have also done a factory reset on the phone and nothing has worked. Any ideas. I really love this phone and would hate to have to send it back. Here is more info from the phone:
