You don't have to sign up when you purchase. You're given a set amount of days from the date of purchase to add it but I forget the exact amount of time. I wanna say 15 days but I could be wrong.

Yep, 30 days and you can add it via your My Verizon web page, if you have that set up.
 
let's say I add the Asurian coverage and the the phone is lost/damaged 3 months later, I assume I just pay the $99 deductible and get a refurbished phone(or a new one, if I'm lucky)

will the replacement phone still have Asurion coverage as long as I keep paying the monthly fee?

Yes, but you have two claims within a year, I believe. I am a fan of SquareTrade as Coffeeman says. Get the warranty with a 30% off code (Google for one) and an even better deal.

EDIT: I just checked SquareTrade and there is a $99 deductible. A two year plan is $99.

Hmmm...Square Trade must have just changed the deductible to $100.00...my plan still shows $50.00 deductible.

I love SquareTrade and buy it for almost every electronic device that I own.(such as TV, tablet, laptop)

However, SquareTrade does NOT cover loss/theft. Since that's more likely to happen to a phone that I carry around all day long, I feel the Asurion policy is more suited towards me.

Theft and loss for two Samsung Galaxy S Insurance My last coverage was due to theft when I had been to a coffee shop. I had one stolen from my auto but my auto is $500.00 or $1000.00 deductible. I even had to pay the glass breakage but my Asurion after the deductible sent me a new not refurb (lucky one) tablet.

I was recently reviewing my auto policy because I just paid off my auto loan.
Under comprehensive coverage, if I drop from $1000 dedutcible to $50 deductible,
it will only cost me $7.93 per month. (about $96 per year) That's not really a lot of
money... I wonder if I should drop it to $50 deductible. :)

I had my vehicle financed through what used to be GMAC and they would not let me have $1000 they made me take $500. I don't have an answer to your question that would be up to you. Only you know what you can afford. I have learned to just not leave my electronics out where they can be seen from the outside of the car and always lock it no matter where I have it parked. Now if I can teach my wife not to leave her purse where it can be seen from outside the car when she takes her wallet out and goes inside the store to make a purchase. I would purchase the Asurian for my phone as it covers many other things which can and do happen to phones. Just my opinion on it. YMMV

Something to think about on phone insurance. I know some people have their phones insured thru their house policy and maybe their cars under theft. Had a insurance company explain something that most people don't know, if you file a claim for your phone under your homeowner insurance it will go against your claim history and raise rates or cause termination of home insurance, all for $700 claim. Was very highly recommended not to claim your phone's and this particular company charges $10 year for phone coverage, they suggest not to buy the coverage.

No you don't have to sign up for it. Although the salesmen will definitely try to get you to, as well as buy as many accessories as they can show you.

Square Trade is interesting. The only thing they don't cover is if your phone is lost or stolen.
 
If you purchase your phone at Costco they have a deal with square trade for 99 bucks and a $25 deductible you get up to four replacements and a fifth specific to battery. So If you have used four replacements and the battery goes bad on the fourth you are good. I love it. My HTC one was only 182 out the door plus the warranty. Little over twenty bucks more than it would have been at the ATT store without the warranty.

You have a 30 day window from the time of purchase to accept or decline insurance. I would recommend the $6.99 TEC Asurion bundle. It gives you 2 claims a year at a $99 deductible per claim on your S4 and covers lost, stolen, damaged and rooted devices. It also extends your Verizon warranty indefinitely, as opposed to just one year (so long as you keep the insurance).

Yes, you do have to pay the deductible and in this case you will need a police report. Said above you are still only allowed 2 claims in a calender year.
