If a phone has a lock screen and is then encrypted, is there an additional password screen that has to be entered after unlocking the phone every time the phone is accessed?

No. Encrypting the device asks for a password with 6+ characters and at least 1 number. That password will work exactly the same as if you were to setup a "regular" lock screen password, but you now have the benefit of having an encrypted device.
 
For the sake of clarity in my own mind: If I encrypt my Samsung Galaxy S5 I have to use a password, which must contain a number and be at least six characters long, every time I unlock my phone? A PIN is no longer an option even if I'm just unlocking the S5, rather than restarting it from power off?
