what is the site to file a claim I can't find the info for some reason.
Is there a deductable for a loose usb port? Mine is getting loose and my Evo isn't charging properly.
If you have TEP on your Sprint plan, total equipment protection, you can take it to a sprint store where they may look at your phone and see if they can fix it or substitute one out. A long time ago, in a far away galaxy, I did this, but it was closer to September of last year. It was only 3 months out - within the warranty. Sprint might tell you to file an Asurion claim, however, so be prepared for that. Personally I complained alot because at the time, there wasn't a single scratch on my phone and the port wasn't working. Manager came out, asked if I had any trouble before, saw that I recently purchased it, and went and got a clean, crisp new one for me. I got to keep the box, microsd card and everything.
There was someone ealier in this thread that said they were told to file an asurion claim - but i think your mileage may vary.

Not necessarily. Everybody on this thread isn't looking for a cheap upgrade, we just answered the question that was asked...I actually had issues with my phone and was told by a Sprint rep to bring my phone in for service or file a claim. Since I was out of town on business for a month, I didn't have the time to take my phone in during business hours so I filed a claim and it just so happened they sent me a 3D. I wasn't expecting it, all I wanted was a phone that worked while I was hundreds of miles away from home.
but I am left without a phone for a couple days :-\
fwiw, Asurion uses overnight shipping.
