Flashing ROMs on sch i545

I haven't been able to upgrade from kit kat to lollipop on my phone. Every time I try to do it, I'm given the message that the upgrade was unsuccessful. AND when I try to install a new rom, I get pretty much the same message. I have rooted and unrooted and rooted again and no matter what, they simply won't take.

What am I doing wrong?

Easiest way to upgrade in your situation is to download Samsung Kies and do the update through there. Make sure to have you serial number and model number written down because it will ask you for them.
