Glass screen protector

Ok been searching the forum but the ones that were suggested I can't find anymore. Had one I loved and did its job as few weeks ago and I fell backwards on my phone. Now I can't find one that's like the one I had. Bought another it literally is too large and I wrote the company to let them know. Now I'm on the hunt for a new one. I'd like it to look like this.

Any help is appreciated. The longer portion is for the bottom buttons. Not sure why it turned it sideways.

In order to help you out - Report what kind of Phone is that. Normally is the
Phone carrier at whom you subscribed that carry the original Screen
glass protection for your phone. Also get it from any store that
sell Cellphone accessories. They installed for customers
