GNex going into bootloader screen. Is this normal? (Screen shot)

My wife's phone powers down sometimes (she has all of the known issues with signal strength and battery drain). When you power it up you sometimes see the following. Anything I should know? Product name - tuna??

The only reason I can think that would happen is if you're holding down the volume down button accidentally while pressing power to turn it on.

I know I wasn't holding down any buttons and she would have no reason to do it either.
