Green android icon replaces icons

I have seen this question here several times, and it seems no one can answer. About 4 months ago, I had several app icons on my home screen turn into the green Android icon. When this happens, if I select one of those icons, I get a message that the application is not installed.

If i select the white button at the bottom of my screen, those apps are not found. Is that the app drawer? The name of the app does not change on the home screen. It is different apps each time this happens.

When this happens, the android icons will appear for a week or two, and all of a sudden my tablet is back to normal.

I believe this is happening after I install a new app from Google Play. Right now, I cannot open Google Play.

This is crazy and very frustrating. Your help is greatly appreciated.
 
Which phone you have? Are you using a launcher , and does phone have SD card that capable to move apps to it? Also if you press play store what exactly happens?
 
Welcome to Android Central! mustang7757 is on the right track -- this usually happens when an app has been "moved" to the SD card, and then that symlink is subsequently broken (typically due to the SD card being unexpectedly removed, or due to SD corruption).
