What you have there is one nano sim, one nano to micro sim adapter and one micro to standard sim adapter. The Note 5 uses a nano sim so in answer to your question, yes just the tiny piece, that's the nano sim.

Yes -- use the very last one since that is the Nano SIM. The other things are just adapters (as "mhunter" said) and you don't need them for your Note 5.
