Home Screen Image Randomly Resets

Ever since I got the Android 5.0 update on my T-Mobile S5; the background for my home screen randomly reverts back to the default. Am I the only one this is happening to; and if not, how do I fix it?

I downloaded a new wallpaper and saved it on my device. It hasn't happened since.

I wonder if my problem has to do with using a photo that was no longer saved on my phone.
