Hot Swap not possible

Hi,

Lately, I've been getting some random message about "Hot Swap" which from Google means to switch your storage or whatever without turning your device off. Well that's all good and great, but this message pops up every time, so long as my phone has the SD card in it, it happens and doesn't stop even after I change the storage area.

The pop up just tells me to change default storage location. There's two choices, Default Storage (which is about 1gb) and then there's the SD card (which is about 14-16gb), but I have the option to cancel all though this means the popup will occur a few minutes later.

So, how do I get rid of this error? I think I might have to replace the SD card itself, but nothing's wrong with it, I don't think. It's been in my phone since the day I received it (brand new from Amazon) and has never been taken out of the phone ever since.

Please help!
