How do I format my Samsung S4 Active?

I have tried formatting from phone...no laptop, but it won't format. I have no clue what I'm doing wrong. I searched the forums and they are all for different phones. Does the disk need to be in phone to format? I'm just lost. PLEASE HELP!!!!!

It's a new card. Have read directions from everywhere....says I can format from my phone. Followed directions...didn't work.
