How do I reinstall an app that was once installed on old sd card?

Cannot reinstall app that was once installed on old sd card

The SD card in my galaxy tab 3 went bad, and now several apps show gray icons with a small sd card icon over the app's icon. These apps do not show as being installed (Either in application manager or the play store), so I cannot uninstall them. When i try to redownload the app from the play store, the play store crashes as soon as it starts downloading.

Any ideas?
