How do you get rid of YApps on Galaxy S4?

This Yapps app found its way on my sister's Samsung Galaxy S4 a couple of days ago and we can't get rid of it because we can't figure out what's the real name of the app.

It doesn't appear in the installed apps list in the Settings menu.

The app has put on a lock screen that pulls ads. It also displays full screen ads whenever the phone has a data connection.

Factory reset did not get rid of it. See screenshots below:

I've emailed the guy but he hasn't replied yet. Is there an app or a way to uninstall this?
