how to reformat sd card for phone camera

My phone had to be rebooted and now my camera doesn't work.
It says "unknown SD card.
I think I should reformat the card but I don't know how ?
Please help ?

I think you should buy a new one.

If you can't find a format choice in recovery, put the card into a USB adapter and format it in a computer. But, as chanchan05 said, it may be shot. It may also be counterfeit. SD cards are something you should NOT try to save money on by buying cheap ones.
