How to restore the vanished phone icon?

Galaxy S4 with Android 5. I can receive calls but without the icon I can't make them. Did I delete some vital Samsung component?
 
No. Go into the app drawer, find the app (it's named Phone), long-press it and drag it to the home screen. Then long-press it and drag it down to the dock. (You may have deleted the shortcut that was in the dock, but that's not a big deal.)
 
My problem is that it's not in the app drawer, or in its customary place in the dock at the bottom of the home screen. However there are three instances of it under All in the Application Manager. Each has a different data total (1.8, 5.59 and 8.8 MB), and I hesitate to Force stop two because the warnings look alarming. I did clear the caches and data of two and disabled them, but the phone app icon is still absent from view although clearly the phone's function is present and able to receive calls and dial out after a missed call. Afterwards it again disappears, so that the only way to start a new call is from a listed contact.
 
The solution now found is to restore the Contacts from App Mgr Disabled. I had disabled them thinking the Google Contacts would do. The Google Contacts icon is round with a blue background, while the Contacts needed by Phone has a square reddish icon. Both show a head and shoulders.
