So here's the story since I can't think of anything simpler to say. Since I rooted my phone, I decided to get ROM Toolbox from the Play Store. Using ROM Toolbox, I downloaded the Cyanogen Mod. Then I backed up my stock ROM first to the SD Card and then tried to flash Cyanogen. Now my phone gets stuck on the CyanogenMod boot screen and I don't know how to go back or fix it. I tried going into HBOOT and TW recovery but nothing seems to help. Also while I'm in TW recovery, my SD Card doesn't want to mount so I can't flash any ROM from there. Now I have no idea what to do. Any help will be greatly appreciated thanks.

Sounds like you forgot to flash the Boot.img via fastboot.
