I got the weird SD Card pop-up again...

So some of you probably remember I made a recent post about a weird popup I got when unlocking my phone. Well, it came back and I not only carefully read it, but I also took a screenshot. Is my SD card failing or is my phone infected with some nasty malware? I will try removing and dusting my SD card, but I highly doubtful that is the issue since my phone has a waterproof/dustproof rubber cap covering the SIM/SD slots. My phone is a CAT S41.
After I clicked "no", it proceeded to reload my launcher. I have no idea why the launcher task got killed. What is really weird is that the popup dialog looks like the dialog box style from Android 4.2.2 / CyanogenMod 10.1.3 . I know there is some legacy code still in Android (sim toolkit , at least on Nougat, looks like an Android 2.x app), but I really would not expect legacy code in the SD card management part of Android.

Personally I suggest that you get a new SD card as soon as possible. It sounds like the SD card you have is failing.
