Intermittent Google Assistant - OK Google

Not sure what the problem is but Google assistant (Ok Google) doesn’t always work when I say “Ok Google”, has anyone else been having this same problem, if so is it just happing to Moto G5 Plus phones or across the board with other devices.
I do have Action Launcher installed, not sure if this why it doesn’t always work, I keep retraining the voice.
Is there a way to start fresh, like opt-out and then opt back in to see if that corrects the problem?

same as mine and my old dtek 60. it was all over the place. when it works it's good but when it doesn't it's spectacularly fails. i ask it to call my wife Janine and it insists on calling a work colleague called Julie. try as I may she just won't call the wife.
