Is my SD Card bad?

So, I just had a fun experience tonight with my phone deciding it wanted to pseodo-reset itself. Decided it wanted to forget most of my synced account information (FB pictures, some google contact info), some system settings, and all my apps that I had moved to my SD Card. This isn't the first time this has happened, either, nor is it limited to the ROM/kernal. The first time this happened that I remember was on ACE 2.0 (though I can't remember which kernal I was using then), and now with SFR using the Genocide kernal. I'm just wondering if this is something that can be caused by SD card issues.

Also, on a similar note, does anyone know where the data for apps stored on the SD card is located? Wouldn't mind being able to back that up.
