LG G4 H811 from T-Mobile Sim Unlock

Same situation here...the Device Unlock app just returns an error on my pre-release T-mobile LG G4....

Sorry to bring up an old post, but has anyone found a solution to this issue. I had an unlocked international version of the G4 which got stuck in a boot loop and was returned to LG who were kind enough to replace the phone, however, that's the end of the good news. The phone returned is locked to T-Mobile, which renders it useless on my home network and a phone call to T-Mobile was of no help either as they "cannot" unlock it through the app because it was never registered on their network.

Any help/suggestions would be greatly appreciated.

Exact same thing happened to me. Bought my phone off ebay here in Canada, boot loop...sent to LG, now locked again. I factory reset it a couple of times, and then restarted it. When it asks to load the previous user data, I chose no then was able to use the t-mobile unlock app to unlock. (previously when I went into the sim app it said I was permanently unlocked, even though t-mobile said I was relocked to them.) I accidentally hit temporary 30 day unlock...so hopefully it will let me do a permanent unlock after 30 days.
