Media volume goes MAX?!

Many times today, when I start a video or app or game, even though my phone is in vibrate, AND I checked my media volume before, it goes MAX volume! This seems to especially happen with ads. Is it a coincidence? I restarted my phone once and it fixed the problem. But now it happened again.

I am pressing the volume down button when the video is loading. It's really annoying, especially at work or in bed

I hope someone has an answer for this. My Galaxy Note 5 does the same thing, it is annoying. It can even go to max in the middle of a game/video from low.

Happens to my coworker too. I think it's happening when I unplug headphones or disconnect from Bluetooth! Test to confirm

I'm going to post on the Google Product forums/group page so it will actually get looked at by a Google official/support person. Or maybe I'll find something on the group related to this issue
