My Note 4 from Verizon has crashed and damaged 3 new sd cards in less than a year, what should I do?

note 4 from verizon has crashed and damaged 3 new sd cards in less than a year

i have noticed that if i transfer files from my pc to my sd card, it makes my card crash and phone wont detect it, i have lost all pictures and apps everytime this happens, i have been able to reformat it a couple of times until it finally cant be formatted nor read anymore, i've never had this type of issues with any prior samsung phones i've owned.
this happened prior to updates to newer version as well as after updates.
