My SD card reads empty and has unsupported files. How do I fix it

Are you able to format it from the storage section of the settings on your phone? Formatting/erasing on the phone will ensure its properly formatted for the phone to read it.

First put the card (not the phone - the card in a USB adapter) into a computer. Any files? Copy them to the computer. Then follow what dirtydantx said. (If the files are corrupt versions of something you need, just formatting the card will delete them.)
