Need to Flash phone back to verizon

I received a Fascinate phone from a relative. Her son had the phone flashed over to Metro PCS. I used Odin and followed the instructions for DI01 and everything seemed to work okay.. until you dial *228 to reactivate the phone.. it still calls Metro PCS.

What do I need to do to get this phone flashed completely back to Verizon service and usable again?
