Why is my N5 get heat while I'm using it? I just do a simple task like open apps, listen to music?

There are maaaaannnnnyyyyy possible reasons. Many.
Is your SIM good? Are your signals good? Have you done any app Cached Data clearing of late, or Cache Partition wipes, which might help?

Mine over heats after about 15 minutes of music or game playing. I just let it cool off. My galaxy s4 did the same thing.

Dial *#*#4636#*#*, click on Battery Information. What does the temperature reading say?

During light use (texting, web browsing, etc), my battery temp is between 90-95 deg F. Gaming causes it to rise to 100 often.

How to know if my Sim is good? I cleared cache, and wiped it today

"Heat is work and work is heat", as physicists assure us. There will always been some heat produced when you are using the CPU, whether for Camera or any other app. I just checked my temperatures...about 90°F battery, and about 104°F for CPU.
My own personal temperature I assume to be 98.4°F, and I feel fine. Probably your phone is fine too if the temperatures are not much higher.
