Not sure where the issue is...

Pixel 6 Pro and Garmin watch. I get notifications on the watch when the phone is in do not disturb mode but I don't get them on the phone. Clearly the phone has to be sending them to the watch, but why?

Do not disturb only effects the phone.
The text, phone call, email still comes in, just muted on the phone. Is the same type of setting available on the watch?

You can choose on the watch app what notifications you want presented. There are some you might want on your watch, but not on your phone, and vice versa. I guess there's some logic to that.

Solved. Needed to enable do not disturb on the watch as well.
