Note 5 Camera lens broken

Has anyone had this problem? I don't know how this happened?. My phone was in a good case.

Yes, My camera rear lens just cracked for no reason.
And i try to remove the broken glass and i think, i have scratched the camera lens and is not focusing the if a far picture is taken. But still works if i take a picture few feet away.

I need to replace the camera lens and cover glass.

Not sure exactly how to take the phone cover off, as i try just a little and that is glass also, so go figure what happen! it start to crack also.
