Notification sounds only coming through on bluetooth headset

Galaxy S10 seems to play the notification sounds only through the headset when it's connected. When it's not, the sounds work just fine. So if my headset is on but not in my ear, I don't hear any notification sounds. Anyone else seeing this issue?

Same issue on note 9 after software update. My work phone is an iPhone and it does this too which really pissed me off and made me glad that I use android for my personal phone.. But now.....

I believe it's an 9.0 Pie issue. Well, not issue, but it's the way sound settings work with the new version.

Reviving this old thread. Same issue now that I'm on Android 10. Notification sounds only play through headset when connected via Bluetooth. The only exception I've seen is with the Ring app. Ring notifications still play through the phone speaker even when I'm connected to headset or car via BT. There must be something the devs wrote into the Ring app that force this behavior.
