On my Galaxy S10, my "Quick Decline Messages" if the call comes from a "SMART" phone.

When the call is from a "SMART" phone, my only options are to Block or Report - or answer, of course.

Are you using a launcher other than One UI?

Are you using a third party dialer/phone app?

Do you ever get the Decline option? Is there anything assumed or known to be different about those calls? You've kind of muddied the waters mentioning SMART phone, does that mean that the calls you cannot decline are from mobile devices or the ones that are not?

I have an S10+ and I always get Accept or Decline for every call, so I'm trying to figure out why you are not getting the decline option. I used to get options to send a message, but now that I think about it I don't recall seeing that option for a while. It's just odd that your only options are Accept, Block or Report... In your Phone app settings there is an option to reduce the number of Spam calls you get, it's the second option when you open settings Caller ID and spam protection, do you have this enabled? Perhaps if it detects that the call might be spam is why you're getting Block and Report???

No where in all of my Phone app settings do I see a way to add or remove Decline, unless it is part of Answer automatically. But that doesn't explain the Accept button being present... Answer the questions posed and we'll go from there.

I do not know what a launcher is. I do not use a third-party dialer. The calls that I cannot decline are from callers identified with the SMART appellation.

The launcher is basically what opens apps, provides settings and gives your device an aesthetic appearance. Many people change their launcher to get more settings options and customize the look of their phone.

The only app I can find in the Play Store called SMART is a business interface of some kind. Is this something you installed or was it pre reinstalled. Either way is there more to the name to help me look it up or find it?

You could reset your settings. If it's something you accidentally turned on resetting your settings could fix the whole thing.

To that go to Settings-General management-Reset-Reset settings.

You won't lose and data but your timers, display settings and sounds settings will be taken to default along with other system settings.

Perhaps this is a UK thing. My S10 has the SMART whatsit which will identify the number and caller if it's a commercial call. I'm pretty sure I can hit the red telephone to end the call

Ok because I couldn't find it, but I am on the US system. Still though there seems unreasonable to think that there is no way to decline the call. If you're seeing a red and green phone symbol then I would think that the red symbol would decline the call. Galactic Zoo can you confirm, just for my knowledge, that you see both during all incoming calls?
