A server error, perhaps. What does your network provider say?

Same phone, same exact issue. I uninstalled the Google App updates, rebooted and it works fine now. Settings > application manager > Google App.

Just to be clear, in my situation all voice commands were not working. "Can't reach Google" and "sorry I didn't catch that" depending upon the app.
