Normally you can't flash normal recoveries by .zip. recoveries are not .zip files. they are .img files. you need to download flash image gui.apk in order to flash it. Also, I can almost guarantee you the problem is not in the recovery. You don't have to install factory rom.

I am trying to install the factory ROM but I couldn't figure out how to do it with this screwy COT recovery.
Why are you trying to install the factory rom???????? If you want to go back to complete stock, you will have to use kdz to flash it with a computer. that'll also change your recovery back to stock.
what do you mean a screwy recovery?? there is nothing wrong with the recovery, its something you are doing.
I am trying to go back to the factory ROM because I am getting a setup screen that connects me to Sprint. I have a VM phone. I know for a fact that I installed a VM ROM, so I have no idea why it is doing this. And the reason I don't like COT is because it is totally different from the standard recovery. It is much easier to find explanations online of how to do things with the standard recovery than it is with this COT recovery.
When I try to install a different recovery the information on the screen races by and you can't read any of it and then COT just goes back to the previous menu. It doesn't display any of the information on the screen like the other recoveries so you have no idea what happened. I don't understand what is so great about this recovery.
Again. Use flash image gui to get a new recovery. And that makes absolutely 0 sense. Out of the approximately thousands of times quattrimus has been flashed, you are the first person with this problem. You must be doing something wrong.

Hi Andrew - do you still have this zip file you linked to? I'm trying to repurpose my optimus v as a mp3 player and I made the same mistake as the poster here.
cot recovery seems to be all dead links.

just don't use it to flash post gingerbread roms. break your bootloop, reboot once. flash mirage for your mp3 player, it's best and most stable.
This is the latest version of COT that I found. The ZIP installer contains COT variations for the LS670, MS690 and VM670. FYI:
