phone camera only working from screen side, no more reverse button

the phone was working just fine this morning, went to use it this afternoon and when i go to take a pic, i can only see myself .... plus the reverse button is missing........... help, please

ps, i have tried so many things to correct this issue.... battery out, off/on, clear cache, etc
