My wife's S4 has been having the same issue since the last system update a few weeks ago. Today I had it happen to my S4. I would think that this is a sw glitch that needs to be resolved by the developers. I remember all the hell we went through over the previous update, and I am not going to jump through the factory reset etc. due to the fact that others are having similar problems. This needs to be solved by the developers of the OS.

Just noticed thread #13 above from THEPRFCT10. Deactivated the "Increase-ring-volume-when-device-in-pocket" option and so far the problem has not recurred. Factory reset (which I mistakenly allowed the Best Buy Samsung rep to do) did nothing except cause me other configuration issues which I am still trying to correct. Why, oh why do these OS authors come up with these questionably needed features, fail to test them adequately to detect bugs and then ignore forums like this when the issues are pointed out?

Yes! Deactivated the "Increase-ring-volume-when-device-in-pocket" option and it worked. Makes some sense when you think about it. The designed feature turns volume up when it goes in your purse or pocket (how do it know?) and down when it comes out.
This has been happening on my S4 for some time, prior to the release of the update (which I ACCIDENTALLY installed this morning, much to my sorrow).
I, too, thought that perhaps the exceedingly bad placement of volume (exactly opposite the on/off button) was leading to accidental changes to the ring tone volume, but after being extra careful to pay attention when switching the phone off, that doesn't seem to be the case. Its just random, and is totally unrelated to the new version of Android. On MY phone, at least.

I'll check that and see if it makes a difference - though right now my phone is in partially dead mode pending factory reset after accidental installation. Im actually going crazy from this. My ringtone keeps getting set to the lowest and my notifications going to earpiercingly loud. I'd also love to know where this setting I can change is as I'm about to go buy a fricken iphone!

It is happening on my S4 Mini. I have tried numerous reboots and visited numerous forums. I am in Canada using Rogers...They are my next call.
I experienced the same thing on my and my wife's S4's. It would happen every day after 1 or 2 phone calls. Did some research. Try unchecking the "increase volume in pocket" option. I did, and haven't had the problem all week. Settings > My Device > Call > Increase Volume in Pocket
PROBLEM SOLVED!! Some updates of the Android operating system set the default to ON for the INCREASE VOLUME IN POCKET. Even if you have your ringer volume is set at the highest level, with this feature turned on the operating system will DECREASE your ringer volume so that it can turn it up in your pocket. EASY FIX: Deselect this option. Here's how: Go to SETTINGS, then select MY DEVICES. Tap on CALL. Then scroll down to INCREASE VOLUME IN POCKET and deselect it. PROBLEM SOLVED!
I have had this problem of the ringtone reducing volume after calls so will try turning off increase volume in pocket ty for the idea have been pulling my hair out with this problem.
I have been having a similar problem and found a working solution. I installed an App "Volume Locker". If you change the volume you have to verify that you want to make the change with an on screen pop up! If you do not verify that you want the change it reverts back to the default or last verified setting. It works for me!

LOLLIPOP SUCKS :
My number one reason why I don't like Lollipop is because it is trying to look like iPhone.
2. NO OPTION TO KEEP KITKAT BLACK BACKGROUND COLORS and avoid Extensive use of white and/or bright pastel screens. Lollipop pastel print with pastel pages makes everything more difficult to read and consumes more battery. Extreme eyesore at night. Please help us out. GIVE US THE OPTION OF THE KITKAT COLOR SCHEME.
3. Smaller keyboard keys. Can't read mini symbols within white key due to light gray print. Thanks for that.
4. 20 Apps running in the background. They do not close when you turn off the phone as they did in Kitkat. When I call text Service to ask why my phone is getting so hot, they tell me to shut down some of my running apps. REALLY! Same as iPhone.
5. Calendar doesn't display small event icons as in Kitkat, only a dot. Why did you eliminate that feature?
6. Orange Text Message Send button (with orange print) too close to message board, easy to accidentally send message.
7. Reduced contact Emojis from square to small circles. How is that better? Oh, I forgot, that's how it is in iPhones.
8. Floating tools. Advantages do not out weigh disadvantages. Uses valuable screen space. Impossible to press the w, x and y key in contacts, without hitting the plus button.
9. Removed many popular printed links to tools in favor of icons that must be learned. Such as edit and groups in contacts.
10. Bluetooth now disconnects and can not reconnect. Must reboot phone or radio. Never happened with Kitkat.
11. Was very comfortable NOT having temporary phone contacts in my "Contacts" Group. I stored them in a temporary group that I created. Now lollipop WILL NOT allow you to uncheck "Contacts" as a group. Why must you change things just for the sake of changing them? How is that an improvement? If it were the other way around [as in now I can uncheck contacts as a group], that would be an improvement!

14. In Kitkat when you press one of your favorite contacts they open to let you choose what you want to do. In the Lollipop down-grade it calls the contact. So what if I wanted to get directions, send a text or something else?
15. When editing a contacts there used to be a minus sign next to all the phone numbers in the contact so youcan delete them. Now you have to tap on the number to get the minus sign to appear. Another step backwards.
16. This one may be a little difficult to understand. But if you have used the battery gauge in Kitkat, you will definitely notice the difference. Battery useage app in Lollipop shows percentage of battery used. That's normal, BUT the percentage breakdown no longer adds up to 100% unless you are looking at the chart with 0% left. In Kitkat it showed you the percentage breakdown based on what is currently used. No matter how much is used, the breakdown always added up to 100%. That way you were able to accurately tell that the screen is consuming 50% of the battery without taking out a calculator. EXAMPLE; In Lollipop you look at the chart with 90% left. The percentage of screen useage will probably show 2%. In this case the screen did not use 2% of the battery it used 2% of its estamated total capacity. In KitKat, it shows the percentage each item used of the 10% of battery used.
17. Ringtone volume defaulting to 10% after receiving a call, even though I have set it to 90%. Now every time I receive a call, I have to go into setting and reset the Ringtone volume. PROBLEM FIXED: had to turn off INCREASE VOLUME IN POCKET. Apparently that doesn't work anymore.
The longer I have lollipop the more I hate it. If I am unable to find a new Samsung phone with Kitkat, then instead of getting Lollipop, (the iPhone copycat want a be)... I MAY AS WELL GET THE REAL THING, an iPhone. I have seen many people switching from Android to iPhone. I don't know anyone who switched from iPhone to Android. GOOD JOB GOOGLE, YOU JUST HELPED APPLE STOCK GO UP A LITTLE MORE. Keep up the good work

Feel a LITTLE better that others are having the same problem but still VERY upset about this problem never being fixed by any of the software "developers". Having got that off my chest, can anyone please tell me where the "Increase ring volume when device in pocket" option can be found? My software is Android 5.0 (lollipop) and, again, my phone is a Samsung Galaxy S4. I've checked in the "Settings", "My Device", "Sound & Notification" and can't seem to find anything in any of these options. I've bounced around in some other settings but again, can't find this "pocket" options anywhere so if someone could please help, I'd appreciate it! Hopefully this will help my problem of ringtone getting automatically turned down until the next great, UNdeveloped software is forced on to us! Thanks anyone for your help!!

Go to SETTINGS, MY DEVICE, CALL and scroll down to INCREASE VOLUME IN POCKET. Uncheck the box!
You might start a NEW THREAD with your comments about Android 5.1 I agree with most of your comments but after using the new upgrade I like it more! It is hard to see in bright light or outdoors and does need options to change the theme or screen colors! Some new THEMES would be just great!

*** SOLUTION FOUND !! *** worked for me !!
I was having the exact same problem... put the phone in my pocket... later take it out... and unknown to me the volume goes down and I start missing calls..
look under (settings) .... (Call).... (increase volume in pocket) ... UNCHECK this box
not sure if it was here or under a different website.. but it was explained this way.
if checked, the phone turns the volume UP when it goes in your pocket... (maybe, hopefully) like it says it will... what it does not say and really does do.. is when you pull it out.. it turns it down...
uncheck the box and try it.. worked for me, no more missed calls and every time I have looked for the last week volume was up full amount where I keep it !
