Samsung galaxy s5 camera help!

I have a brand new tmobile s5 and when I take a picture and go to gallery the picture comes out fuzzy (like a broken TV or monitor) and when you go back to the gallery it shows just a black screen and Grey square.

Also when you take video it pops up saying file not supported! I have a 128gb memory card (which I wiped and formatted) what can it be? And how do I fix

The problem could be related to the SD card. Have you tried another or storing on the internal?
