Yup, that's definitely a launcher issue. It used to happen to me a lot on GoLauncher.

i'll agree with everybody else.... this looks like a launcher issue.... change launcher, update it, or reinstall....

i actually use the Apex (free), and it sometimes gives me some trouble while trying to rearrange icons on home screen or add widgets where are other icons/widgets already located...
