Screen turns off

Ok, so my phone is doing this thing where when the screen is on it will turn off when I pass my hand over the front camera and will then turn back on when I move my hand away. It does this intermittently and when I power the phone off and then on again it goes away only to return anywhere from a come days to a couple weeks later. Anyone else have this occur? I'm running stock on an AT&T G3. Any help is greatly appreciated!

This is what phone should do during calls, camera detects that phone is near the ear and shuts down the screen to preserve battery.. It is strange to have this activated all the time :/

The camera is not what makes this work, its the proximity sensor.
Check your settings for related actions.
