SD Card eon't automatically mount

So, whenever I restart my phone the SD card is unmounted. If I go I to settings I can mount it there, but that 's getting really old really fast. Anybody know what's going on?

It's one of two things...well there are other possiblilites but lets start here

1) the card is having a problem.. reformat the card IN the phone.
2) phone is having software isses itself. When was the last time after the ICS update that you reformated the phone?
