sd card help

I'm new to the android world and was wondering if I have a bad sd card or what.
On my note 4 I transferred some of my apps to the SD card and when I restarted my phone all my pictures and videos were gone and the apps that were saved to the SD card are now grey. When I click on the app It says not installed. Also it does not recognize my sd card anymore. So I remove the sd card and re-install it and it says it's blank and needs to be reformatted.
I'm so confused. Please help. Is my sd card corrupt

Ugh. Samsung has a bit of a history of SD issues (at least, that's how it seems judging by posts on these forums).

Try removing the SD card and inserting it into your computer's SD reader. Does it show up at all? If so, backup as much data as you can to yoru computer hard drive, then run chkdsk to look for any bad sectors on the SD. http://forums.androidcentral.com/am...guide-using-chkdsk-fix-corrupted-sd-card.html
 
If you bought the card from an online retailer (including Amazon) you most likely have a fake card. Most of the failed card problems I have seen here is because of cards bought on Amazon. You should only buy from local retailers. If the card came from online try to backup to your computer. If you can see the image files move them to the computer. Once done trash the card and buy one locally and format it in the phone before attempting to place files on it..
 
I buy my cards on Amazon, and have no issues with them.
Up until a year or so ago, SanDisk had a problem.. They rewrote the internal ROM code, and things are OK now.
Don't buy cards from some rinky dink seller on eBay.
