SD Card Related Problems

I recently bought a HTC Incredible to replace my old phone. The phone works fine until I put the SD card in. Once the card is loaded into the phone, android.process.media force closes when anything to do with media tries to open, and I mean from pictures and music and when the ringtone goes off for a phone call or text, and when I try to set the alarm, and some others. Another problem I'm facing is the phone recognizes that there is a SD card and it is mounted, but, it wont let me see the pictures or music, let alone the "cool ringtones", all I have are the stock ringtone and they wont go off if the SD card is in the phone because of the android.process.media failing when sound try to load. I have formatted the SD card and it didn't help, I have restarted the phone every time I set something new or made a change and so far nothing has worked. Anyone have any ideas???
