SD card repeatedly disconnecting

I have a Samsung Galaxy S8+ and have recently noticed that throughout the day my phone will repeatedly disconnect and reconnect the micro SD card.

I noticed this begin after the last time I transferred pictures off of my SD card and onto my computer. Afterwards my phone started giving me a "Connect to transfer media from SD card" notification despite the fact that I had just removed 90% of it. After a while that notification stopped appearing and now the phone constantly gives me "SD card not connected" and "SD card unexpectedly removed".

So far I've only tried removing and reinserting the SD card to make sure it was seated properly but that hasn't helped. Need to know whether I should reformat the card, buy a new card, or contact T-Mobile about a defect with the phone itself.
 
First, power the phone off, then remove the SD card. Wipe the contacts with a clean piece of linen (a handkerchief will do). Replace the card.

If that doesn't work, power down again and connect the card itself directly to the PC. (You might have to get a USB adapter for it.) If that doesn't work, the card is bad - replace it. (Spend the extra few dollars for a SanDisk card - the cheap ones are guaranteed for 5 years, the better ones carry a lifetime [yours] guarantee. And you can get online help at SanDisk microSD Tech Support)

If the card isn't bad, back up everything in internal storage (see Backing up an Android Device) and contact T-Mobile. The SD card port is probably intermittent.
