I have had my HTC sensation 4G rooted for a bit now, and I needed to send it in for repair so I needed to unroot it.

I followed the guide on theunlockr for unrooting the HTC sensation 4G.

After doing
adb reboot-bootloader

fastboot oem writesecureflag 3

fastboot reboot-bootloader

The phone only boots into that screen, I have tried and searched for a bit. Not really sure what to search for though.

At the top it says
*** Security Warning ***
Pyramid PVT SHIP S-ON RL
HBOOT-1.17.0008
Radio-10.11.9007.15_M
eMMC-boot

Thanks for any help you can give!

Did you had a custom ROM on it? Looks like you unrooted with the custom ROM still on it. S-ON tells you that you successfully unrooted, but it can't find the original .img to boot. You need to restore the stock ROM before unrooting. For step by step instruction and how to restore your semi-bricked phone take a look here. Step 5 is what you're looking for. I hope this helps.

If anyone else had that problem ever, turning S-ON before installing a stock ROM, you can also head to Welcome to HTCdev and follow the instructions there, takes about ten minutes and your device will be able to reboot into your custom ROM once more.

What if you are not rooted and you accidently wiped your boot image ? I wiped my data , cache, etc , without making a backup and now my phone is stuck on the dre beats when I try to start it , what should I do ?

Had the same problem..Follow these steps..

1st. Go to HTC Dev.com and unlock your bootloader...
2nd. After unlocking your bootloader(warranty will be void), download the juopnutbear S-OFF utility from Unlimited.IO.
3rd. Install the utility and the Go to CMD and run C:\juopunutbear:>controlbear.exe with your phone connected to your PC via usb cable..
4th. After that install clockworkmod recovery.(pyramid 5.0.2.0)for HTC Sensation/4g/XE.
5th. Reboot your phone..

There you go...you are officially S-off..
