My wife's Fascinate has never been rooted and is completely stock. She got it right before the official froyo was released, and got the ota update. The baseband version and build number both read EB01 Though. I have read here that EB01 was never released by Verizon. Does this mean her phone was hacked by a malicious app?

If it truly reads EB01 then it is not stock, and did not receive an OTA, because as you said, EB01 was never an official build. An app cannot make those changes, however. It would have had to have been flashed to that firmware and radio
I read about some apps that root the phone and steal your data once they are installed. I thought this may have happened. I am now remembering right after the ota update her phone was locking up and freezing. I tried to odin sc 2.9.2 to the phone, but it didn't take. Odin said that it was successful,but the phone just rebooted to stock. After that her phone was working fine so I left it stock. I guess I am gonna have to go back to eclair and ota again.
