Welcome to Android Central! Is this your phone and your SIM (not "sims") card? Did you ever put a SIM lock on it? If it's someone else's phone or SIM card, did they put a SIM lock on it?

If it's your phone and your card, then check the package your SIM card came with, and look for a PUK code there. Otherwise, contact your carrier and ask for help.
