If so, does anyone have a copy of it?

Yes it does, I have copy but its on TB.

The stock "My Account" was simply a web link and I don't remember it ever working, which is why it was always removed.

The app you want is called "Minutes Checker: Virgin Mobile" by Jay Goel
There is also one that will let you make payments, but I don't know anything about it.
